Case Manager

We are seeking a full-time Case Manager. The Case Manager will be reporting to Visions and Pathways’ (V&P) Director of Behavioral Health.

Responsibilities include:

  • Provide and maintain case management case load in accordance with the guidelines set forth in the case manager job description.
  • Conduct new client intakes.
  • Coordinate services for V&P clients according to treatment plan (therapy, psychiatry, medical, DCP&P, family contacts, UCM, etc.).
  • Keep files up to date with treatment information.
  • Maintain and document a minimum of two contacts weekly with residents.
  • Maintain and document a minimum of one family contact bi-weekly.
  • Develop client treatment plans (bi-weekly plans for shelter residents) & participate in monthly & quarterly meetings.
  • Crisis de-escalation, as needed.
  • Attend court hearings, IEP meetings, initial psychiatric appointments, and other community meetings as they relate to specific cases.
  • Transport clients as needed.
  • Help ensure appropriate staff coverage in V&P residential services programs.
  • Maintain case records and ensure compliance with licensing standards.
  • Help ensure life skills education services are provided and completion of Ansell-Casey life skills assessment, AIF, RHYMIS, HYMIS, billing reports, LOS and case management reports monthly and quarterly.
  • Maintain statistical data regarding clients and prepare year end stats.
  • Follow guidelines as outlined in Rules for Child Care Workers and all program manual policies and procedures.
  • Ensure that discharge planning occurs and is well documented.
  • Inter-department communication and planning to ensure comprehensive service delivery to agency clients.
  • Demonstrate understanding and respect for diverse cultures, adapting communication styles to effectively interact with individuals with various race, sex, and gender identities.

Qualifications:

  • Bachelor’s degree and three years of applicable experience required. Graduate degree in a relevant field is a plus.
  • Valid NJ driver’s license is required.
  • Excellent organizational, computer and communication skills.
  • English/Spanish Bilingual candidates preferred.
